We make sure all our new patients feel welcome, safe, and taken care of. During your first visit, a physical therapist will review your medical history and take time to listen to your concerns and expectations. To determine an accurate diagnosis and develop a treatment plan, they will perform a multitude of observations and analysis following a thorough process specific for our patients. From there, the physical therapist will explain any issues and prescribe a sustainable treatment for your rehabilitation and functional improvement. We typically provide a detailed home exercise program and management plan before your next appointment.

For a shorter time in the waiting room, please fill out the New Patient Forms and bring them to your first visit. You can also fax the forms to 517-306-2372, or send in regular mail to 13060 S US 27 STE C4, Dewitt MI 48820.  Due to the sensitivity of patient intake information, emailing the intake paperwork is not safe practice at this time.
